Job summary

Optum Home & Community Care, part of the Optum family, is expanding its Senior Community Care program to coordinate care for high-need patients in nursing homes, senior housing and assisted living settings.

As an Advanced Practice Clinician in SCC, you will perform comprehensive assessments, manage short-term rehab, document accurately, and collaborate with an interdisciplinary team to improve access to care.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state.
• Active and unrestricted driver's license with access to reliable transportation.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire (as required).
• Ability to gain a collaborative practice agreement where applicable.
• Ability to navigate varied environments and work with the interdisciplinary team.

Responsibilities

• Conduct comprehensive assessments including medical history, med reconciliation, vital signs and exam.
• Support members during a short-term sub-acute rehabilitation episode.
• Ensure accurate and complete documentation including ICD-10.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide counseling and education to patients and caregivers.
