Job summary

Optum Home & Community Care in Fayetteville, NC is seeking a Nurse Practitioner or Physician Assistant to join the Senior Community Care team. You will perform comprehensive assessments, document findings, and coordinate care for patients in nursing homes, senior housing, and assisted living settings.

This role involves independent work, travel between care sites, and a focus on transition of care, patient education, and accurate ICD-10 coding.

Qualifications

• Graduate of an accredited MSN program for NP or accredited PA program.
• Board certification for NP or PA as specified.
• Active, unrestricted license in the state of North Carolina.

Responsibilities

• Perform comprehensive assessments and document findings per CMS regulations.
• Ensure diagnoses are ICD-10 coded and documented to support visit diagnoses.
• Provide care coordination and initial medication review.
• Travel between care sites as required.
• Participate in SCC quality initiatives and trainings.
