Optum Senior Community Care (SCC) seeks a Certified Nurse Practitioner or Physician Assistant for the SNF Patient Connect program in Seattle. You will support Medicare/Medicaid patients with transitional care and care coordination in SNFs.
This autonomous role collaborates with physicians, nursing staff and case management to complete comprehensive health assessments, close care gaps, and document visits. Expect travel to SNF facilities within a designated area, professional growth, and impact
Qualifications
- Active NP or PA licensure in WA and board certification as NP or PA.
