Position Overview

The Nurse Chronic Care Coordinator, Remote, performs telephonic encounters with patients on behalf of our partners each month and develops detailed care plans within our care plan templates in the electronic health record. This position begins as an Independent 1099 Contractor but offers the potential to transition to full-time W2 employment with benefits.

Responsibilities

• Conduct telephonic patient encounters and develop care plans.
• Manage a patient panel with a minimum of 100 patients per month.
• Complete encounters on 90% of assigned patients.
• Follow the plan of care and practice orders.
• Provide prevention and intervention for multiple disease conditions through motivational coaching.
• Develop positive interactions with patients.
• Contribute to revenue growth by creating billable CCM episodes and increasing management visits.
• Attend scheduled meetings, including bi-monthly staff meetings and monthly one-on-ones.

Qualifications

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
• Current RN / LVN / LPN license with no disciplinary actions.
• At least two years of clinical experience in Med/Surg, Case Management, or home health care.
• Experience with Electronic Medical Records and proficiency with Windows or Mac systems, Microsoft Office 365, Teams, and Excel.
• Ability to work independently with initiative, judgment, and excellent organizational skills.
• High-speed internet connection; Windows 10 or higher or Mac system (no Chromebooks or iPads).
• Excellent verbal, written, and listening communication skills.
