Position Overview

The Nurse Chronic Care Coordinator, Remote will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our care plan templates in the electronic health record. This is initially an Independent 1099 Contractor position with potential to transition to full-time W2 employment with benefits.

Qualifications and Requirements

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
• Current license to practice as an RN / LVN / LPN with no disciplinary actions
• At least two years of clinical experience in Med / Surg, Case Management, or home health care
• Experience with Electronic Medical Records and proficiency with Windows and Microsoft Office 365
• High-speed internet; Windows 10 or higher or Mac (no Chromebooks or iPads)
• Excellent verbal, written, and listening skills

Responsibilities

1. Conduct telephonic patient encounters, developing and implementing care plans
2. Manage a patient panel of at least 100 patients per month, completing encounters on 90% of assigned patients
3. Follow HIPAA privacy laws and maintain confidentiality
4. Attend scheduled meetings and participate in team communications
5. Provide preventive care and intervention through motivational coaching
6. Document interactions thoroughly and accurately
7. Adhere to the plan of care and practice orders

Compensation and Benefits

Pay rates vary based on the number of encounters, from $10.00 to $16.00 per encounter. Payment tiers require two months of consistency. Encounters typically take at least 20 minutes each. Part-time hours are flexible, with a minimum of 20 hours per week.
