Job description

Join our mission to help transform healthcare delivery from reactive, episodic care to proactively managed patient care that prevents life-changing problems before they happen for patients with two or more chronic conditions. We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.

Position Overview

The Nurse Chronic Care Coordinator, Remote, performs telephonic encounters with patients on behalf of our partners each month and develops detailed care plans within our care plan templates in the electronic health record. This is initially an Independent 1099 Contractor position with potential to transition to full-time W2 employment with benefits.

Responsibilities

1. Conduct telephonic patient encounters monthly.
2. Develop and update care plans within our templates.
3. Manage a patient panel of at least 100 patients per month.
4. Complete encounters on at least 90% of assigned patients.
5. Follow the plan of care and practice orders.
6. Provide prevention and intervention for multiple conditions via motivational coaching.
7. Engage positively with patients and improve revenue through billable CCM episodes.
8. Attend scheduled meetings to define scope of work.

Qualifications

1. Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
2. Current RN / LVN / LPN license without disciplinary actions.
3. Minimum two years of clinical experience in Med/Surg, Case Management, or home health care.
4. Experience with Electronic Medical Records and Microsoft Office 365; HIPAA compliance required at home.
5. Strong initiative, judgment, organization, and problem-solving skills.
6. High-speed internet and a Windows 10 or higher PC or Mac (no Chromebooks or iPads).
7. Excellent communication skills.
