Job description

We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.

Position Overview

The position of the Nurse Chronic Care Coordinator, Remote will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our care plan templates in the electronic health record. This begins as an Independent 1099 Contractor position but offers the potential to reach full-time W2 employment (with employee benefits).

Who We Are Looking For

Esrun Health is seeking Nurses to work part-time from their home office while complying with HIPAA privacy laws. You will set your own hours and will not be held to a daily work hour schedule. You will be contracted to work a minimum of 20 hrs/week. We want our team members to have the flexibility to balance work and home life. Part-time team members will typically dedicate 20-30 hours per week to their assigned patients, choosing what days and hours to work.

Responsibilities
  1. Manage a patient panel based on skill and efficiency, with a minimum of 100 patients per month.
  2. Complete encounters on 90% of assigned patients.
  3. Perform telephonic patient encounters, develop care plans, and document interactions.
  4. Follow the plan of care and practice orders.
  5. Provide prevention and intervention for multiple disease conditions through motivational coaching.
  6. Develop positive interactions with patients.
  7. Contribute to revenue by creating billable CCM episodes and increasing management visits.
  8. Attend mandatory meetings, such as bi-monthly staff meetings and monthly one-on-ones.
Compensation

We pay $10.00 per completed patient encounter up to 99 encounters/month, increasing to $16 for over 250 encounters/month. Payment tiers require 2 months of consistency. Each encounter takes at least 20 minutes, including review, calls, documentation, and care planning.

Qualifications
  • Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
  • Current license with no disciplinary actions.
  • At least 2 years of clinical experience in Med/Surg, Case Management, or home health care.
  • Experience with Electronic Medical Records and Microsoft Office 365 in a HIPAA-compliant home environment.
  • Strong judgment, organization, time-management, and problem-solving skills.
  • High-speed internet; Windows 10 or higher or Mac (no Chromebooks or iPads).
  • Excellent communication skills.
