NON-COMPACT STATE - PT Remote CCM/RTM Care Management Nurse (CA)

Position Overview

The Nurse Chronic Care Coordinator, Remote will perform telephonic encounters with patients monthly, developing detailed care plans within our electronic health record system. This position starts as an Independent 1099 Contractor but offers potential for full-time W2 employment with benefits.

Responsibilities

• Conduct telephonic patient encounters and develop care plans.
• Manage a patient panel of at least 100 patients per month, completing encounters on 90% of assigned patients.
• Follow the plan of care and practice orders.
• Provide prevention and intervention for chronic conditions through motivational coaching.
• Attend scheduled meetings and maintain detailed documentation.

Qualifications

• Graduate from an accredited Nursing program (LPN, LVN, RN, BSN, etc.).
• Current RN/LVN/LPN license with no disciplinary actions.
• At least two years of clinical experience in Med/Surg, Case Management, or home health.
• Experience with Electronic Medical Records and proficiency with Windows/Microsoft Office.
• High-speed internet and a Windows 10+ or Mac computer (no Chromebooks or iPads).
• Excellent communication skills.

• Ability to recognize warning signs of health deterioration.
• Strong documentation and organizational skills.
• Self-directed with good time management.
