Job description

Join our mission to help transform healthcare delivery from reactive, episodic care to proactively managed patient care that prevents life-changing problems before they happen for patients with two or more chronic conditions. We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.

Position Overview

The Nurse Chronic Care Coordinator, Remote, performs telephonic encounters with patients on behalf of our partners each month and develops detailed care plans within our care plan templates in the electronic health record. This is initially an Independent 1099 Contractor position with the potential to transition to full-time W2 employment with benefits.

Who We Are Looking For
  • Registered Nurses (RN, LPN, LVN, BSN, etc.) with a current license and no disciplinary actions.
  • At least two years of clinical experience in Med/Surg, Case Management, or home health care.
  • Experience with Electronic Medical Records and proficiency with Windows or Mac systems, including Microsoft Office 365 and Teams.
  • Ability to work independently, exercise judgment, and manage time effectively.
  • Reliable high-speed internet connection.
  • Excellent communication skills.
Responsibilities
  1. Conduct telephonic encounters with patients, developing and implementing care plans.
  2. Manage a patient panel of at least 100 patients per month, completing 90% of assigned encounters.
  3. Provide prevention and intervention for multiple chronic conditions through motivational coaching.
  4. Attend scheduled meetings to stay aligned with team goals and scope of work.
  5. Ensure compliance with HIPAA privacy laws.
Compensation and Work Schedule

Pay varies based on the number of encounters completed, starting at $10 per encounter and increasing with volume. Work hours are flexible, with a minimum of 20 hours per week, allowing you to choose your schedule.
