The Nurse Chronic Care Coordinator, Remote, performs telephonic encounters with patients on behalf of our partners each month and develops detailed care plans within our care plan templates in the electronic health record. This is initially an Independent 1099 Contractor position with the potential to transition to full-time W2 employment with benefits.
Pay varies based on the number of encounters completed, starting at $10 per encounter and increasing with volume. Work hours are flexible, with a minimum of 20 hours per week, allowing you to choose your schedule.
