Job description

Join our mission to help transform healthcare delivery from reactive, episodic care to proactively managed patient care that prevents life-changing problems before they happen for patients with two or more chronic conditions. We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.

Position: Nurse Chronic Care Coordinator, Remote

This role involves performing telephonic encounters with patients on behalf of our partners each month and developing detailed care plans within our electronic health record system. Initially, this is an Independent 1099 Contractor position with potential to transition to full-time W2 employment with benefits.

Qualifications and Requirements:
  • Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
  • Current RN / LVN / LPN license with no disciplinary actions.
  • At least two years of clinical experience in Med / Surg, Case Management, or home health care.
  • Experience with Electronic Medical Records and proficiency with Windows or Mac systems, Microsoft Office 365, Teams, Excel, etc.
  • Ability to work independently, exercise judgment, organize, and manage time effectively.
  • High-speed internet connection; system requirements include Windows 10 or higher or Mac (no Chromebooks or iPads).
  • Excellent verbal, written, and listening communication skills.
Responsibilities:
  1. Conduct telephonic encounters and develop care plans as per templates.
  2. Manage a patient panel of at least 100 patients per month, completing encounters on 90% of assigned patients.
  3. Provide prevention and intervention for chronic conditions via motivational coaching.
  4. Attend scheduled meetings and adhere to the practice’s care plans and orders.
  5. Develop detailed care plans to support prevention and intervention.
  6. Contribute to revenue growth by managing billable CCM episodes and increasing management visits.
Compensation:

Pay rates vary based on the number of encounters per month, starting at $10.00 per encounter, with increases up to $16.00 for over 250 encounters. Payment requires two months of consistency. Each encounter takes approximately 20 minutes, including all related tasks.
