Job description

Join our mission to help transform healthcare delivery from reactive, episodic care to proactively managed patient care that prevents life-changing problems before they happen for patients with two or more chronic conditions. We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.

Position: Nurse Chronic Care Coordinator, Remote

This role involves telephonic encounters with patients on behalf of our partners each month and developing detailed care plans within our electronic health record system. Initially, this is an Independent 1099 Contractor position, with potential to transition to full-time W2 employment with benefits.

Who We're Looking For
  • Licensed Nurses (RN, LVN, LPN, BSN, etc.) from an accredited school, with no disciplinary actions.
  • At least two years of clinical experience in Med/Surg, Case Management, or home health care.
  • Experience with Electronic Medical Records and proficiency with Windows or Mac systems, including Microsoft Office 365.
  • Strong organizational, judgment, time-management, and problem-solving skills.
  • Reliable high-speed internet connection; Windows 10 or higher or Mac required.
  • Excellent communication skills.
Responsibilities
  1. Perform telephonic patient encounters and develop care plans.
  2. Manage a patient panel of at least 100 patients per month, completing encounters on 90% of assigned patients.
  3. Follow the plan of care and collaborate with healthcare providers.
  4. Provide preventive care and interventions for multiple conditions through motivational coaching.
  5. Attend scheduled meetings to stay aligned with team goals.
Compensation and Schedule

Pay ranges from $10 to $16 per encounter, based on volume, with increases after two months of consistency. Each encounter takes approximately 20 minutes. You set your own hours, with a minimum of 20 hours per week, and flexibility to choose your workdays and hours.
