Zusammenfassung

Elite Home Health in Florida is seeking a Registered Nurse to provide and coordinate skilled nursing care in patients’ homes. The RN conducts comprehensive assessments, develops and implements care plans, and ensures care aligns with physician orders, Medicare Conditions of Participation, and agency policies.

The RN will supervise LPNs and CNAs, perform SOC and OASIS assessments, and document in the EMR while traveling within the assigned territory. Weekend availability is required.

Qualifikationen

• Graduate of an accredited School of Nursing (ASN or BSN) and Florida RN license in good standing.
• Minimum of 1 year nursing experience; home health or hospice preferred.
• Current CPR certification required.
• Proficient in OASIS assessments and complete documentation accurately.

• Perform comprehensive patient assessments and start of care visits.
• Develop, implement and update individualized care plans.
• Coordinate care with physicians, therapists and the inter-disciplinary team.
• Complete accurate EMR documentation and regulatory compliance.
