Job summary

SnapCare in Grand Rapids, MI is seeking an experienced RN for a night shift position to conduct nursing assessments within 8 hours of admission and develop individualized care plans. You will supervise LPNs and CNAs, document care, and participate in team conferences, ensuring patient progress and safety.

Minimum two years of bedside experience is desired; BLS is required before the first shift, ACLS for CIRH is also required before the first shift.

Qualifications

• Minimum two years of bedside clinical experience.
• BLS certification required before first shift.
• ACLS required prior to first shift for CIRH.

Responsibilities

• Perform nursing assessments within 8 hours of admission to develop individualized plans of care.
• Supervise Licensed Practical Nurses, Certified Nursing Assistants, and/orientees.
• Document all assessments, interventions, and patient responses accurately and timely in the medical record.
• Provide patient information and status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences.
• Initiate discharge planning from day of admission and provide patient teaching in anticipation of discharge.
