SnapCare in Beachwood, OH seeks a Registered Nurse for a night shift in a critical illness and rehabilitation setting. You will contribute to a multidisciplinary team delivering high-quality patient care.

You will perform assessments within 8 hours of admission, plan care with patients and families, document thoroughly, and participate in discharge planning from day one. The role requires strong communication and supervision of support staff.

Qualifications

• Minimum two years of bedside clinical experience.
• BLS and ACLS certifications required before first shift.
• Active RN license required.

Responsabilités

• Perform nursing assessments within 8 hours of admission to develop individualized plans of care.
• Identify teaching needs and goals related to each patient's diagnosis with patient/family and interdisciplinary team.
• Maintain accurate, timely documentation of assessments, interventions, and patient responses.
• Initiate discharge planning from day of admission and provide patient teaching for discharge.
• Communicate changes in patient status to attending physician and Charge Nurse to ensure continuity of care.
• Provide direction and supervision of ancillary nursing staff as needed.
