Job summary

SnapCare is seeking a Registered Nurse for a 13-week assignment in Johnstown, PA. The role covers 12-hour night shifts from 7:00 PM to 7:00 AM and requires BLS and ACLS before the first shift.

The nurse will work rotating weekends/holidays and perform comprehensive patient assessments and care planning on a dedicated rehabilitation-focused unit. Ideal candidates have at least two years of bedside experience and will participate in discharge planning and interdisciplinary team conferences, while

Qualifications

• Minimum two years of bedside clinical experience highly desired.
• BLS certification required prior to first shift.
• ACLS certification required prior to first shift.
• 13-week assignment.

Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission.
• Develop and implement individualized plans of care based on patient assessment data.
• Provide patient information and status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences.
• Document all assessments, interventions, and patient responses accurately and timely in the medical record.
• Participate in patient rehabilitation program on assigned unit.
• Utilize nursing process of assessing, planning, implementing, and evaluating patient care.
• Initiate discharge planning from day of admission and provide patient teaching in anticipation of discharge.
• Communicate pertinent information regarding changes in patient status to attending physician and Charge Nurse.
• Provide direction, training, and supervision of ancillary nursing personnel.
• Delegate appropriately using problem-solving techniques and prioritization.
• Assume charge responsibility following orientation period as requested.
