SnapCare invites applications for a Registered Nurse to join our Cleveland, OH facility. This night shift role focuses on comprehensive nursing assessments, care planning, and collaboration with an interdisciplinary team to ensure high-quality patient care.

Responsibilities include supervising LPNs/CNAs, documenting care, and leading discharge planning. BLS is required before the first shift; ACLS required for CIRH. A minimum of two years of bedside experience is highly desirable.

Qualifikationen

• Two years of bedside clinical experience preferred.
• BLS certification is required before first shift; ACLS required before first shift for CIRH.
• Night shift position.
• Active nursing license in the relevant state may be required.

Aufgaben

• Perform nursing assessments within 8 hours of admission to develop individualized care plans.
• Supervise Licensed Practical Nurses, Certified Nursing Assistants, and orientees.
• Document assessments, interventions, and patient responses in the medical record promptly and accurately.
• Provide patient information and status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences.
• Participate in patient rehabilitation using the nursing process (assess, plan, implement, evaluate).
• Initiate discharge planning from day of admission and educate patient and family.
• Communicate status changes to attending physicians and Charge Nurse to ensure continuity of care.
• Delegate and supervise ancillary nursing personnel; provide direction and training as needed.
• Assume charge nurse responsibilities after orientation period as requested.
