Job summary

Qualifications

• Active RN license in the state of practice
• Excellent communication with residents, families and health care team
• Ability to develop and implement nursing care plans

Responsibilities

• Provide resident care by reviewing physician orders and communicating the nursing care plan
• Develop and revise nursing care plans based on resident condition and assessments
• Educate residents and families on care requirements, medications and self-care
• Evaluate care outcomes against standards and adjust plans as needed
• Maintain safe, infection-control compliant care environment and proper medication handling
• Coordinate admissions, discharges and transfers within the facility
