Christian Living Communities | Cappella Living Solutions seeks a Night Shift QMAP (Personal Care Partner II) in Grand Junction, CO. Qualifications

• Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
• Must be able to read, write and speak the English language
• High school or Equivalent (GED)
• One year experience in elder care preferred
• Strong self-management and organization skills
• Strong multi-tasking abilities with flexibility and adaptability to change
• Ability to handle difficult and sensitive issues and maintain confidentiality

Responsabilités

• Attends to residents' personal care needs and assists with ADLs as directed
• Provides personal care such as bathing, dressing, meals, laundry with attention to resident needs
• Identifies and reports resident care needs to supervisor
• Responds to family members' needs as appropriate
• Maintains open communication with supervisor and peers
• Monitors residents' safety and well-being
• Reports suspected abuse, neglect, or misappropriation of resources
• Reports changes in resident status to supervisor
• Monitors and documents resident conditions
• Assists with prescribed daily medication routine as appropriate
• Alerts RN/LPN to issues with medication records/supply
• Ensures timely documentation including refusals
• Restocks medication carts and storage areas
• Completes work orders for replacements or repairs
• Participates in monthly staff meetings
• Other duties as assigned
