Job summary

Christian Living Communities (CLC) – Cappella Living Solutions (CLS) is seeking a QMAP Care Partner (FT) for Wednesday–Saturday, 7:00 AM–3:30 PM in Grand Junction, CO. This role handles medications under nurse supervision, assists residents with ADLs, and supports daily care in a values‑driven environment.

Qualifications

• Medication certification per state requirements (QMAP‑A/L, Med Tech, Med Aide, Nurse Delegation)
• Must be able to read, write and speak English
• High school diploma or GED or equivalent
• One year of elder care experience preferred
• Strong self-management and organization skills
• Good multitasking abilities with ability to handle deadlines
• Ability to handle sensitive issues and maintain confidentiality

Responsibilities

• Provide daily personal care to residents in alignment with mission and values
• Supervise medications under licensed nurse supervision and review MARs
• Assist residents with ADLs (bathing, dressing, meals, laundry)
• Communicate with supervisors and peers about resident needs
• Monitor resident safety and well‑being; report changes promptly
• Document resident condition and medication refusals timely
• Restock medication carts and supplies; complete work orders as needed
• Attend monthly staff meetings and contribute to resident care discussions
• Other duties as assigned
