Responsibilities

• Introduce self and role to patient at start of shift.
• Provide water and assist with activities of daily living as needed.
• Respond to call lights and address patient needs promptly.
• Assist with bed bathing, oral care, and hygiene.
• Transport patients and equipment as directed.
• Assist with patient transfers and room preparation.
• Record vital signs and input/output as directed and notify RN of abnormalities.
• Maintain clean, safe unit environment and comply with infection control policies.
• Document patient information accurately on forms.
• Communicate patient concerns to nursing staff and advocate for patients.
• Participate in department CQI indicators and ideas for improvement.
