Qualifications

• High School Graduate or GED/HSED.
• Team members will maintain annual educational requirements for hospital certifications (e.g. DNV, CARF or others).

Responsibilities

• Assists RN with daily patient care: bathing, oral hygiene, feeding, ambulation and transfers.
• Measures and documents vitals and weight; records data in EMR accurately.
• Performs rounding and communicates observations to RN.
• Supports admission, transfer, and discharge processes; prepares room and handles belongings.
• Collects urine, stool, and sputum samples and conducts point-of-care testing as indicated.
• Maintains stock for nursing area; cleans equipment and work areas.
• Assists with patient positioning, transfers, and nutritional needs under RN direction.
• Maintains skin integrity and uses prescribed patient supplies and devices.
• May remove IVs, provide post-mortem care, and perform bladder scans as required.
• Adheres to infection prevention protocols and uses appropriate PPE.
• Demonstrates age-appropriate care for patients across lifespans.
