Job summary

Advocate Health is seeking a Part-Time Nursing Assistant to support RN staff in night shifts (6:30 pm–7:00 am) with rotating weekends/holidays at our Libertyville facility. Responsibilities include patient care, vital signs, documentation, admissions/discharges, and coordination with departments.

Requires GED/HSED or High School Diploma and BLS within 45 days, plus Nurse Aide in good standing. Position involves assisting with daily patient activities, meals, ADLs, and ensuring patient safety.

Responsibilities

• Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation with use of assistive devices and proper positioning.
• Measures and documents vitals and daily weights; documents accurately in Electronic Medical Record (EMR).
• Participates in admission, transfer and discharge processes under RN supervision; sets up rooms and orients patients.
• Performs purposeful rounding; communicates observations and needs to RN; assists with positioning and transporting.
• Assists with nutritional needs and diet preparation; maintains skin integrity and uses required patient supplies and devices.
• Collects specimens and performs point-of-care testing as indicated; may assist with IVs and post-mortem care as directed.
• Coordinates tests and treatments; maintains knowledge of diagnostics, scheduling, and EHR systems; processes daily patient charges as required.
• Facilitates communications and screens calls; troubleshoots department equipment; arranges repairs as needed.
• Prepares chart forms for admissions, transfers, discharges; verifies patient demographics and registration.
• Maintains positive patient relations and supports a harmonious work environment; adheres to age-specific care requirements.
