Job summary

Qualifications

• Must hold an Ohio LPN license and be in good standing.
• High School diploma or GED required; BLS certification.
• Familiarity with Ohio Nurse Practice Act and standard nursing practices.

Responsibilities

• Performs initial, ongoing and functional health status assessments and documents data from patients and families.
• Develops and coordinates care plans and medical management with the health care team to meet patient needs.
• Evaluates and documents response to nursing interventions and revises plans as needed.
• Delegates tasks per state statutes and supervises delegated work by unlicensed staff.
• Participates in process improvement and champions safety, quality and efficient patient care.
