Job summary

OhioHealth in Ohio is seeking a licensed practical nurse (LPN) to provide comprehensive nursing care to patients and families across the health-illness continuum. The role supports the health care team and adheres to the Ohio Nurse Practice Act. Night shifts include 6p-6:30a, 6p-6:30a, and 3p-1a on listed days.

The LPN will assess patients, document findings, coordinate care plans, and collaborate closely with providers to ensure quality outcomes. Ohio licensure and BLS are required.

Qualifications

• High School or GED is required.
• Graduate from an accredited practical nursing program.
• Current Ohio LPN license is required.
• Must have completed Ohio Board of Nursing approved medication course.

Responsibilities

• Perform initial, ongoing and functional health status assessments and document data.
• Collaborate with providers and healthcare team; communicate findings effectively.
• Develop and coordinate patient care plans and intervene as needed.
• Evaluate response to nursing interventions and adjust the plan accordingly.
• Delegate tasks to unlicensed personnel and supervise as appropriate.
• Participate in process improvement and patient advocacy activities.
