Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess needs, coordinate care with the IDG, and maintain primary clinical oversight for assigned patients during nights and weekends.

The role emphasizes direct patient care, care plan development, and education for patients and families, with a focus on after-hours coverage and compliance with hospice standards.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person hospice care during after-hours shifts, including visits and admissions.
• Assess physical, emotional, psychosocial, and spiritual needs of patients and families.
• Coordinate with the IDG to update care plans and ensure interdisciplinary communication.
• Develop and revise Hospice Plan of Care with physicians and IDG members.
• Educate families on disease progression, symptom management, and hospice philosophy.
• Maintain timely clinical documentation and ensure survey readiness.
