Job summary

Gentiva Hospice is seeking an Afterhours RN Care Manager to deliver high-quality hospice care outside standard hours. You will assess and coordinate care during nights and weekends, maintain clinical oversight, and support the IDG with care planning and documentation.

The role requires an active RN license, 1+ year of relevant RN experience, and strong communication with families and physicians. Comprehensive benefits are provided.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments, admissions, routine visits, and live discharges.
• Assess physical, emotional, psychosocial, and spiritual needs; update plans of care.
• Support the IDG with chart reviews and interdisciplinary communication.
• Maintain accurate clinical documentation and care plans.
• Manage scheduling adjustments and on-call visit triage.
• Ensure infection control standards and regulatory compliance.
