Gentiva is seeking an Afterhours RN Care Manager to deliver high-quality hospice care outside standard hours. You will assess, coordinate, and oversee care for patients and families during nights, weekends, and on-call periods, acting as a key member of the IDG.

You will educate families, manage care plans with physicians, ensure compliance, and maintain documentation. Strong nursing experience in hospice or related settings is essential.

Competenze

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Mansioni

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Develop and revise the Hospice Plan of Care with physicians and IDG.
• Educate patients and families on disease progression and symptom management.
• Maintain accurate clinical documentation including assessments and care plans.
• Support administrative and compliance activities including workflow audits and survey readiness.
