Qualifications

Responsibilities

• Assess individuals' hopes and desires and provide supports to achieve them.
• Help individuals develop relationships in the community and with natural supports.
• Assist individuals in participating fully in the community according to their interests.
• Protect and promote the rights of those served; support advocacy efforts and use personal advocates.
• Provide emotional, physical, and medical support as needed.
• Foster positive, person-centered relationships and promote self-determination.
• Contribute to assessment and planning processes and develop individual plans.
• Implement plans, document progress, timesheets, incidents, and goal completion.
• Complete daily progress notes and maintain communication logs for appointments and goals.
• Assist with day-to-day home operations (housekeeping, meals, laundry, shopping, etc.).
• Drive and provide transportation as required; assist with emergency procedures and safety drills.
• Assist new staff with orientation, mentoring, and training.
