Responsibilities

• Assess each person for hopes, dreams, and goals.
• Support people in developing community relationships and natural supports.
• Assist participation in community activities aligned with the person’s interests.
• Ensure the rights of each person are protected and promoted.
• Assist in developing advocacy skills and participating in advocacy efforts.
• Provide support to meet emotional, physical, and medical needs as required.
• Document progress, goals, and outcomes; complete required forms and logs.
• Assist with day-to-day living tasks (housekeeping, meals, laundry, shopping).
• Drive and travel as required; arrange transportation when needed.
