Responsibilities

• Assess each person’s hopes and desires and provide supports to facilitate achievement.
• Support people in developing relationships in their community and with natural supports.
• Assist participation in the community consistent with the person’s interests.
• Protect and promote the rights of each person; assist with advocacy and personal advocates.
• Provide emotional, physical, and medical support as needed.
• Document progress through daily notes and logs; track goals and outcomes.
• Assist with day-to-day home operations: housekeeping, meals, laundry, shopping, and maintenance.
• Provide transportation as required; drive and travel for services.
• Support safety: emergency procedures, drills, and safety practices in community settings.
• Assist with staff orientation, mentoring, and training; participate in required meetings.
• Overnight sleepover at a residential setting may be required.
• Maintain policy compliance and complete required documentation.
