Job summary

Mercy Home Health is seeking a Clinical Nurse II to lead a Care Delivery Team and direct nursing care for a designated patient group. The CN II demonstrates clinical leadership, coordinating care across assessment, planning, implementation, and evaluation while precepting new staff and ensuring high-quality patient outcomes.

The role is a full-time night shift (7:00 pm – 7:30 am) with every other weekend and holiday.

Qualifications

• Graduation from a state-accredited school of nursing.
• Current licensure from the state of Ohio as an RN.
• Successful completion of general and unit-specific orientation and hospital probationary period. Completes Orientation Checklist.
• Obtain/maintain a minimum of 12 contact hours of continuing education annually which includes Fire Safety and CPR Re-certification. ACLS is required.
• Must have theoretical knowledge combined with clinical experience in providing nursing care for a variety of patients. Must be able to communicate well verbally and in writing with people of varying backgrounds.

Responsibilities

• Direct care for a team of patients with independent assessment, planning, implementation, and evaluation of nursing care.
• Precepts new employees as assigned.
• Directs and coordinates nursing care for a specific group of patients.
• Supervises Licensed Practical Nurses and Patient Care Assistants.
• Works full-time with schedule: 7p-7:30a, every other weekend and holiday.
