Job summary

Illumination Health + Home is seeking a Medical Coordination Assistant to ensure seamless medical care forhomeless clients discharged from hospitals, with follow-up coordination and social care in Recuperative Care Centers. Shift: Sunday-Thursday, 11:00pm–7:30am; pay: $20–$24/hr plus swing and graveyard differentials.

Qualifications

• Basic computer skills with ability to email and summarize data in spreadsheets.
• Bilingual in English and Spanish.
• Proficiency in Google Suite (Mail, Docs, Sheets, Calendar).
• Valid CA Driver’s License.

Responsibilities

• Perform initial client intake and ensure proper discharge planning with continued care.
• Coordinate follow-up appointments and medications after hospital discharge.
• Link clients to medical services (insurance, PCP, specialists) and arrange transportation as needed.
• Maintain HIPAA compliance and accurate documentation in medical records and logs.
• Provide health education and support to clients to promote overall wellbeing.
