Job summary

Illumination Foundation is seeking a Medical Coordinator to deliver high-quality care for patients experiencing homelessness in recuperative care settings. The schedule includes weekends and nights with a pay range of $30.00 - $35.00 per hour, plus $1.50 swing and $3 graveyard differentials.

The position involves night hours from 11pm to 7:30am on Saturdays and Sundays and requires handling intake, care planning, and coordination of services.

Qualifications

• Current and non-restricted LVN license in California.
• 2 years of relevant experience.
• High School Diploma or equivalent.
• Valid CA Driver’s License.

Responsibilities

• Perform initial intake and establish care plans.
• Assess health status and review medications.
• Coordinate home health visits and follow-up appointments.
• Promote health literacy and patient education.
• Arrange transportation to medical appointments.
• Maintain documentation in EHR and progress notes.
• Ensure HIPAA compliance and infection control.
