Job summary

Illumination Health + Home in California seeks a Medical Coordination Assistant to ensure the seamless medical care of homeless clients discharged from hospitals and contracted agencies. You will coordinate follow-up care and social services in recuperative care centers.

The role requires coordinating transportation, documenting visits, and promoting health literacy, with a 7am - 3:30pm Sunday-Thursday schedule and hourly pay of $20-$24.

Qualifications

• High School Diploma or equivalent.
• Basic computer skills and ability to summarize data in spreadsheets.
• Preferred Medical Assistant Certificate or EMT license; bilingual in English/Spanish.
• 2 years of relevant experience is preferred.
• Valid California Driver’s License.

Responsibilities

• Perform initial intakes to ensure proper discharge from hospital with continuation of care.
• Assess medical and mental health status at arrival with head-to-toe assessments and vitals.
• Coordinate follow-up appointments, medications, and referrals.
• Arrange transportation for medical appointments and document all findings.
• Enter client data into KIPU and maintain logs; ensure HIPAA compliance.
