Job summary

Illumination Health + Home is seeking a Medical Coordination Assistant to ensure smooth medical care for discharged homeless clients at recuperative centers and shelters. The role focuses on coordinating follow-up care and social support with a Sunday–Thursday, 3pm–11:30pm schedule.

Responsibilities include intake processing, medical status assessments, linking to services, arranging transportation, and documenting all interactions while upholding HIPAA and DHS standards.

Qualifications

• High School Diploma or equivalent.
• Basic computer skills including email and spreadsheets.
• Bilingual in English and Spanish.

Responsibilities

• Perform initial intakes to ensure proper client discharge and document findings.
• Assess medical and mental health status on arrival and arrange follow-up with doctors.
• Conduct morning, afternoon, and evening checks for all assigned clients.
• Link clients to medical services, insurance, PCPs and referrals as needed.
• Coordinate home health visits and follow-up MD appointments.
• Provide health literacy education on illnesses, medications and prevention.
• Exit clients with a summary of medications, appointments and insurance info.
• Arrange transportation for medical appointments as needed.
• Report emergencies to Supervisor and activate appropriate medical response.
• Support recovery by maintaining med room cleanliness and inventory.
