Illumination Health + Home in California seeks a Medical Coordinator to deliver high-quality medical care for patients experiencing homelessness in recuperative care centers and shelters. Many weekend and night shifts are available, with a pay range of $30.00-$35.00 per hour plus swing and graveyard differentials.

The role emphasizes intake, care planning, coordination of services, patient education, and meticulous documentation in the EHR, all while upholding HIPAA and safety standards.

Qualifications

• Current and non-restricted LVN license in California.
• 2 years of relevant experience.
• High School Diploma or equivalent.
• Basic computer skills including emails and spreadsheets.
• Valid CA Driver’s License.

Responsibilities

• Perform initial intake to establish care plans based on discharge information.
• Assess health status and review insurance, and arrange follow ups.
• Set weekly goals and verify progress to aid rehabilitation.
• Conduct shifts and checks to monitor wellbeing post-discharge.
• Link clients to medical services and coordinate home health visits.
• Educate patients on illnesses, medications, preventive care and management.
• Arrange transportation for appointments and report emergencies.
• Enter data into EHR and prepare patient reports for billing.
