Job summary

Illumination Health + Home seeks a Medical Coordination Assistant to ensure seamless medical follow-up for homeless clients discharged from hospitals or clinics. The role runs Sunday to Thursday, 11:00pm to 7:30am, with pay ranging from $20 to $24 per hour plus swing and graveyard differentials.

The position emphasizes patient education, care coordination, and adherence to HIPAA and DHS standards. The successful candidate will have a high school diploma, basic computer skills, and ideally

Qualifications

• Bilingual in English and Spanish is preferred.
• Basic computer skills including email and data summarization.
• Google Suite proficiency is desirable.

Responsibilities

• Perform initial intakes to ensure proper discharge planning from hospital.
• Assess medical and mental health status upon arrival and arrange follow-ups.
• Coordinate home health visits and follow-up MD appointments.
• Link clients to medical services including insurance and PCP referrals.
• Provide patient education on illnesses, medications, and preventive care.
• Exit clients with a comprehensive medication and appointment summary.
• Arrange transportation for medical appointments as needed.
• Report emergencies and escalate to supervisor when required.
• Maintain med room cleanliness and inventory for safety.
