Zusammenfassung

Illumination Health + Home is seeking a Medical Coordination Assistant in Whittier, CA to ensure medical care coordination for discharged clients. You will perform intakes, monitor wellbeing, link to services, and arrange home health visits when needed.

The position runs Sunday–Thursday, 3pm–11:30pm with pay ranging from $20 to $24 per hour, plus swing and graveyard differentials. Benefits and HIPAA/compliance duties are part of the role.

Qualifikationen

• High School Diploma or equivalent.
• Basic computer skills including email and spreadsheets.
• Preferred MA certificate or EMT license.

• Enter client information and case notes into systems.
• Perform intakes and discharge documentation for follow-up care.
• Coordinate transportation and follow-up appointments as needed.
• Monitor client health status and promote health literacy.
