Job summary

Illumination Health + Home is seeking a Medical Coordination Assistant to ensure seamless medical care for discharged homeless clients. The role runs Sunday to Thursday, 11:00pm–7:30am, with $20–$24 hourly pay and swing/graveyard differentials. Responsibilities include intake, care coordination, and health education.

Ideal candidates will have a High School Diploma, basic computer skills, and knowledge of HIPAA; bilingual in English/Spanish and a CA driver’s license are preferred.

Qualifications

• High School Diploma or equivalent.
• Basic computer skills; ability to summarize data in spreadsheets.
• Preferred: MA certificate or EMT license.
• 2 years relevant experience.
• Bilingual in English and Spanish.
• Google Suite proficiency.
• Valid CA Driver’s License.

Responsibilities

• Perform initial intakes to ensure proper discharge planning and document findings.
• Assess medical and mental health status on arrival and arrange follow-up care.
• Coordinate home health visits and follow up MD appointments.
• Provide patient education to promote health literacy under supervision.
• Arrange transportation for medical appointments as needed.
• Enter client data into KIPU and maintain logs.
• Maintain confidentiality per HIPAA and DHS standards.
• Support caregiver coordination and documentation.
