Illumination Health + Home is seeking a Medical Coordinator in California to deliver high-quality care to patients experiencing homelessness. The role supports discharge planning, care coordination, and health education across recuperative care centers and shelters.

The position offers weekends and nights with a salary range of $30.00–$35.00 per hour plus swing and graveyard differentials. Travel and shift flexibility are expected.

Qualifikationen

• Current and non-restricted LVN license in California.
• 2 years of relevant experience.
• High School Diploma or equivalent.
• Valid CA Driver’s License.
• Basic computer skills, including email and summarizing data in spreadsheets.

Aufgaben

• Perform initial intake to establish an appropriate care plan based on discharge information.
• Assess health status and vital signs on arrival; review insurance and coordinate follow-ups.
• Set weekly goals and verify completion to support medical/social reintegration.
• Conduct routine checks to monitor wellbeing after hospital discharge.
• Coordinate home health visits and follow-up on appointments.
• Provide patient education on illnesses, medications, and preventative care.
• Arrange transportation for medical appointments as needed.
• Report emergencies and activate appropriate medical responses.
• Maintain med room and assist with inventory and paperwork.
