Concord Hospital is seeking a Patient Care Coordinator to support new patient registration, scheduling, chart preload, and welcoming responsibilities. The role requires excellent customer service, strong coordination skills, and familiarity with CHMG operating systems.

You will preload medical records, coordinate appointments, and act as a liaison with HIMS to ensure providers have accurate information. Strong communication and organizational abilities are essential for patient-centered care.

Qualifications

• High school diploma or GED required; customer service ability preferred.
• Experience with medical office operations and scheduling is desirable.
• Strong communication and organizational skills are essential.

Responsabilités

• Greets and services all new patient calls seeking a provider and establishing care.
• Interview and screen new patients for convenient CHMG practice location and timely access.
• Preloads and coordinates medical records in the new chart, liaising with HIMS.
• Works independently to foster excellent customer service and a warm welcome.
• Coordinates scheduling with providers to promote a patient-centered medical home.
• Other duties of Patient Care Coordinator.
