Qualifications

• Degree in Physical Therapy from an accredited institution.
• Doctor of Physical Therapy (DPT) preferred or education commensurate with graduation date.
• Physical Therapy License in the employing state.
• Possesses and maintains Basic Life Support (BLS) certification for Healthcare Providers.
• Maintains continuing education requirements as mandated by state professional standards.

Responsibilities

• Provide age-appropriate patient care across the lifespan.
• Perform patient evaluations and develop care plans with evidence-based interventions.
• Set functional goals and monitor outcomes with ongoing reassessment.
• Educate patients and caregivers on diagnosis, progress, home programs and equipment.
• Communicate professionally with patients, families, providers and staff.
• Complete documentation, adhere to billing guidelines and maintain productivity standards.
