Qualifications

• Degree in Physical Therapy from an accredited institution.
• Doctor of Physical Therapy (DPT) preferred or educational requirements commensurate with the date of graduation.
• Physical Therapy License in the state(s) of employment.
• Maintains Basic Life Support (BLS) certification for Healthcare Providers.
• Keeps up with continuing education requirements as mandated by applicable state standards.

Responsibilities

• Provide age-appropriate patient care aligned with departmental policies.
• Perform evaluations and develop care plans including modalities and exercises.
• Set functional goals with patients and monitor outcomes.
• Educate patients and caregivers on diagnosis, progress, and home programs.
• Communicate with providers and staff to coordinate schedules and care.
• Document evaluations, progress notes, and discharge summaries accurately.

Education

Doctor of Physical Therapy (DPT) preferred or educational requirements commensurate with graduation date

Obtained degree in Physical Therapy from an accredited institution

Physical Therapy License

Basic Life Support (BLS) certification for Healthcare Providers

Continuing education requirements as mandated by state professional standards
