Job summary

H2 Health seeks a Float Physical Therapist to support outpatient clinics around the Floyd region. You will evaluate patients, create care plans, and deliver evidence-based treatments across multiple sites.

Qualifications

• DPT degree or equivalent (MSPT/MPT/BPT) from CAPTE-accredited program.
• Current Virginia Physical Therapist license or eligibility.
• CPR certification or willingness to obtain.
• Ability and willingness to travel between clinic locations.
• New graduates encouraged to apply.

Responsibilities

• Perform comprehensive patient evaluations and develop plans of care.
• Deliver evidence-based interventions to improve mobility, strength, balance and function.
• Treat orthopedic, post-operative, musculoskeletal, balance and mobility-related conditions.
• Provide patient care across multiple outpatient locations within Floyd region.
• Collaborate with patients, families, physicians, and interdisciplinary teams.
• Educate on injury prevention, wellness and home exercise programs.
• Maintain accurate documentation of patient progress and outcomes.
• Utilize Scribe AI and Raintree EMR to streamline documentation and management.
• Maintain a manageable caseload with ~10–12 visits/day for full-time clinicians.
