Position Highlights

What You'll Need
  • Obtained degree in Physical Therapy from an accredited institution.
  • Doctor of Physical Therapy (DPT) preferred or educational requirements commensurate with the date of graduation.
  • Physical Therapy License administered in the state(s) of employment.
  • Possesses and maintains Basic Life Support (BLS) certification for Healthcare Providers.
  • Maintains continuing education requirements as mandated by applicable state(s) professional standards.
What You'll Do
  • Age-Appropriate Patient Care: Demonstrate knowledge and skills to provide care appropriate to the age of the patients served, assess data reflective of the patient’s status, and identify each patient’s requirements relative to age‑specific needs.
  • Patient Evaluation and Care Planning: Interpret medical records and provider referrals, complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods, and establish an appropriate patient care plan with evidence‑based interventions.
  • Goal Setting and Outcome Monitoring: Establish functional goals with the patient/caregiver and monitor outcomes to enhance physical and functional abilities, providing individualized adjustments based on the patient’s response.
  • Patient and Caregiver Education: Provide education based on patient needs, address learning barriers, and support patient discharge with written home programs.
  • Professional Communication: Communicate in a professional and timely manner with patients, caregivers, providers, staff, and external customers, and attend multidisciplinary meetings.
  • Documentation and Compliance: Complete required documentation including evaluations, progress notes, education, team conference reports, discharge summaries, and billing charges, and comply with insurance guidelines.
  • Environment: Maintain a clean and orderly department by properly storing equipment and ensuring functionality.
  • Professional Development: Pursue growth through continuing education, in‑services, and review of professional literature.
  • Operational Improvement: Engage in performance improvement initiatives, identify quality and operational improvement opportunities, and manage assigned caseload.
  • Float: Meet Level 1 requirements and practice competently across assigned departments as needed.
Knowledge, Skills & Abilities Required
  • Intermediate computer skills.
  • Excellent communication, interpersonal, organizational, decision‑making, and problem‑solving skills.
  • Ability to perform basic office functions.
  • Comply with HIPAA guidelines and access PHI on a need‑to‑know basis.
  • Successfully complete all competency assessments/requirements pertaining to patient care.
