Job summary

Hugh Chatham Health, Inc. is seeking a part-time Home Care Physical Therapist to manage a patient caseload in the University's City area. The role involves delivering PT services in home settings, coordinating with physicians, and documenting care in the EMR system.

The ideal candidate holds a Doctor of Physical Therapy and an active state license, with a willingness to travel to patient homes and uphold high-quality outcomes. A valid driver’s license and auto insurance are required.

Qualifications

• Doctor of Physical Therapy degree required and licensure eligibility as per state requirements.
• Current physical therapy license in the applicable state (or South Carolina if applicable) and BLS certification.
• Must have a valid driver’s license and auto insurance; willing to travel to patients' homes.

Responsibilities

• Manage daily patient caseload in home care settings with timely care and documentation.
• Collect data from assessments and EMR to determine needs and plan interventions.
• Identify high-risk patients and coordinate with the care team to adjust plans as needed.
• Develop and implement plans of care in collaboration with the physician and supervise applicable staff.
• Maintain communication with the multidisciplinary team and update schedules and patient information.
• Perform ordered PT treatments in compliance with state practice acts and home health standards.
