Job summary

HCA Florida Woodmont Hospital in Florida is seeking a Social Worker (MSW) PRN to perform non-clinical biopsychosocial assessments and supportive interventions on inpatient floors and in the outpatient emergency department.

You will collaborate with RN case managers, physicians, and families to identify discharge barriers, develop appropriate plans for high-risk patients, provide emotional support, and connect patients with financial assistance and community resources.

Qualifications

• Master's degree in social work required.
• Experience in inpatient, crisis, or hospital social work preferred.

Responsibilities

• Perform biopsychosocial assessments and interventions on inpatient floors and ED.
• Conduct crisis intervention and provide emotional support to patients and families.
• Identify discharge barriers and develop appropriate discharge plans for high-risk patients.
• Coordinate meetings with care teams, patients, families, and caregivers.
• Assist with post-acute planning and referrals for financial assistance or resources.
• Collaborate with RN case managers, physicians, and insurers to address patient needs.
• Respond to physician and social work consults across the facility.
