Job summary

HCA Houston Healthcare Tomball is seeking a Social Worker (MSW) PRN to perform biopsychosocial assessments on inpatient floors and in the ED, coordinating discharge and providing emotional support to patients and families. The role works alongside rn case managers, physicians, and insurance teams to address barriers to discharge and ensure patient-centered care.

The MSW uses crisis intervention, counseling, and resource referrals to support post-acute placement and post-discharge planning.

Qualifications

• MSW required; Licensure as LCSW, LMSW, or RCSW-I preferred.
• Experience in inpatient settings and/or crisis response is advantageous.
• Knowledge of discharge planning, community resources, and patient advocacy.

Responsibilities

• Collaborate with the care team to identify patient needs and plan discharge.
• Address barriers to discharge including financial, psychosocial, and medical factors.
• Provide psychosocial assessments, counseling, and resource referrals as needed.
• Support patients and families in decision-making aligned with their values and goals.
• Respond to high-risk situations and coordinate with physicians and nurses.
