Job summary

HCA Florida Osceola Hospital is seeking a Social Worker (MSW) to perform biopsychosocial assessments and interventions on inpatient floors and the outpatient emergency department. The role supports patient and family-centered care, addressing complex discharge planning and resource needs across ICU, Trauma, Oncology, NICU and ED services.

The MSW will collaborate with care teams to identify barriers to discharge, provide emotional support, and coordinate referrals for financial assistance,

Qualifications

• Master's degree in social work or clinical social work field.
• At least 1 year of social work experience.
• Strong knowledge of discharge planning and resource referrals.

Responsibilities

• Collaborate with RNs, physicians, case managers, insurers and staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery including financial, psychosocial, medical and spiritual factors.
• Maintain knowledge of Medicare/Medicaid and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients.
• Coordinate and facilitate meetings between care team, patients and families to support informed decisions.
• Provide psychosocial support, counseling and information referrals to address emotional, social and financial needs.
• Offer emotional support during end-of-life or crisis situations and participate in discharge planning and advocacy.
