HCA HealthONE Swedish seeks a Social Worker MSW PRN to perform biopsychosocial assessments and supportive interventions across inpatient floors and the ED. You will collaborate with RN case managers, physicians, and insurance to support patient discharge and resource needs.

The role emphasizes crisis intervention, family engagement, and post-discharge planning for patients with behavioral health and social needs, ensuring patient- and family-centered care throughout care transitions.

Competenze

• Masters degree in social work (MSW) required.
• 1+ years of professional experience or MSW internship required.

Mansioni

• Conduct biopsychosocial assessments for inpatients and ED patients.
• Perform crisis intervention and discharge planning for high-risk cases.
• Coordinate with care teams to address barriers to discharge and resources.
• Provide emotional support and referrals to financial assistance or community resources.
